THIS AGREEMENT (hereafter “Agreement”) is entered into by and between Boddie Family Boxing Academy, Inc., (hereafter "Academy") and

(hereafter "Service Provider"), an independent contractor

working with the Academy as

NOW, THEREFORE, FOR AND IN CONSIDERATION OF the mutual promises and agreements contained herein, the Academy hires the Service Provider, and the Service Provider agrees to perform services for the Academy under the terms and conditions set forth below.

1. Scope of Services.

The Service Provider has not and will not receive training from the Academy regarding how to perform the services. The Service Provider will ensure that the Services are performed in accordance with all relevant federal, state and local laws and regulations, and will maintain all necessary licenses, permits and registrations required for the services.


The Service Provider agrees to complete all documentation and items required during the hiring process, including but not limited to, fingerprinting, background checks, TB test, and fully understands that successful completion of these items is a mandatory requirement which must be completed prior to providing services for the Academy.

Facility Rules

Welcome to the Boddie Family Boxing Academy! We have worked hard crafting a community that treats each other with kindness and respect. While we provide high-quality equipment and clean facilities, our team members as well as our participants are what make this space special. To ensure everyone has a great workout experience, please review, familiarize and follow these simple guidelines focused on respect, hygiene, and safety.

  1. BFBA management and staff are not responsible for lost, stolen, or damaged personal belongings. We strongly encourage you to secure all items, and to avoid bringing valuable possessions into the facility. There are no lockers at the facility. However, backpacks can be stored under the available benches.


  2. Children under the age of 13 are not permitted to use the cardiovascular equipment, boxing equipment, free weights, or boxing ring without adult supervision.


  3. Questions about practices and/or classes should be directed to the Coach/Instructor, and questions about the BFBA program and/or events shall be directed to BFBA Directors, AJ and Jennifer Boddie.


  4. Don't give unsolicited advice. Only offer workout tips if someone specifically asks for them.


  5. Smoking. BFBA is a no-smoking facility. Smoking inside of (or in the immediate vicinity or grounds of the facility) is strictly prohibited. This policy applies to all forms of smoking, including but not limited to, cigarettes, cigars, pipes, e-cigarettes, or any other tobacco or vapor product. The use of electronic smoking devices, including e-cigarettes and vape pens is also strictly prohibited. This prohibition covers all indoor areas and outdoor areas of the facility, or within 100 feet of any facility entrances, windows, and air vents.

    1. Participants are responsible for ensuring their guests or visitors comply with this No-Smoking Policy. Failure to comply with the No-Smoking policy can result in termination of your membership.
